Animal Owner Assistance

Traditional crowdfunding platforms like GoFundMe, GoGetFunding or FundRazr pay the donations to whomever sets up the account, but FreeAnimalDoctor.org only pays the veterinarian. They personally contact, by phone, every animal owner who is listed on their website, and communicate with their veterinarian, including obtaining medical records and bills. Plus, upon application approval, they'll launch your campaign and work with you, to help promote your campaign.There are absolutely NO Fees, specifically for the animal owner. FAD only asks donors to voluntarily contribute a 10% add-on to their donation to help them cover costs, but this is completely optional. They don’t take any fees out of the donated amount, and they do not charge credit-card processing fees. 100% of your donation is applied to the animal's account!Where P.D.I. Comes In:Once running, we will further your campaign efforts by aiding you and FAD with promoting your campaign on social media (i.e. Twitter, LinkedIn, Instagram, Reddit, Facebook and more). Plus we will actively promote your campaign through other marketing channels, such as our blogs.Additionally, as funding permits, we will donate to your campaign.However, before any of this can happen, we require that at least five requirements are met:1) Your application must be approved by FreeAnimalDoctor.org

Simply repeat your personal information in this section.2) Upon setting up your SCRA account, you've submitted a Single Record Request to obtain a report certifying Title 10 active duty status for provisions under SCRA.3) You've downloaded & saved the SCRA .pdf form populated.4) You've renamed the above .pdf file in the following format:"MilitaryConfirm-LastNameFirstInitial-DDMMYYYY (use the date provided at the top right of the populated .pdf)5) You've emailed said form to: [email protected] with subject line: "MilitaryConfirm-LastNameFirstInitial"6) You've completed & submitted the P.D.I.'s assistance request form.
